I/We understand what the regulations are for the registration of The 15th Annual Fall Spiritual Holistic Expo. Everyone working in my booth must sign this form. Two tickets come with the booth, all others working in my booth will have to pay the daily rate of $5.00.

I/We agree to only do or sell what I have listed on the above listing. I/we will not hold The 15th Annual Fall Spiritual Holistic Expo 2007, Metaphysical Universal Ministries, The Metaphysical Learning Center, St. John’s Church of Faith, Lehigh Agricultural Society Associations, or any Director and/or Officer of either association legally responsible for any loss, theft or injury.

Payment rendered indicates agreement with above terms.
